DFA Apply/ Renew hotline no. 737- 1000 and it was delivered right in front of my doorsteps through DFA Express Passport Delivery w/o any hassle. That was 3 years ago and during that time it was still the GREEN colored passport so there is no more appearance. Since there is a new Machine Readable Passport now then DFA requires applicants for a one-time personal appearance. Online Passport Application or Renewal is just a click away.

My niece got her passport about 12 years ago but was lost and never been used. She cannot recall the nos. of the lost passport. Can she apply for a new one since it was several years ago ehen she was still single?
